04 · Psychosocial Factors

The conditions underneath.

Psychosocial hazards, including work-related stress, excessive workload, fatigue, trauma exposure, occupational violence and poor workplace relationships, are recognised as serious work health and safety risks under Australian law. Organisations have a duty to identify, assess, and control them in the same way they manage physical hazards.

Those conditions rarely stay in their own lane. Fatigue, rostering, supervision, workload and psychological safety turn up as organisational factors in physical incidents, and an investigation that stops at the sharp end will miss them. Our ICAM analysis is adapted to surface them where the evidence supports it, so the corrective actions reach the conditions and not only the task.

Where the question is the state of your psychosocial risk management rather than a single incident, that is a separate piece of work: an independent assessment informed by ISO 45003 and the harmonised WHS framework, or the design of the system itself.

What is an ICAM investigation?
ICAM (Incident Cause Analysis Method) is a structured way of investigating workplace incidents that looks beyond the last action before an event. It sorts contributing factors into absent or failed defences, individual and team actions, task and environmental conditions, and organisational factors. The aim is corrective actions that change the conditions that allowed the incident, not only the behaviour on the day.
Are your investigators qualified?
Yes. Every Soteria 360 consultant who leads an investigation is a qualified ICAM Lead Investigator with frontline experience in high-risk industry. We don't send junior staff to lead your investigation.
Do you investigate psychosocial incidents?
We investigate workplace incidents where psychosocial hazards, such as fatigue, workload, rostering or occupational violence, are contributing factors, and we bring those organisational factors into the ICAM analysis. Can you investigate if our own investigation is already under way?
Yes. We can take over, run alongside your team, or independently review findings already reached. We'll tell you plainly whether the existing work will stand up.
